In consideration of the sales and services by LIV BEE Jewelry Co., their agents, owners, officers, volunteers, participants, and all other persons or entities acting in any capacity on their behalf (hereinafter "LIV BEE Jewelry Co."*), I hereby agree on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representatives, and estate as follows:

 

1. I acknowledge that all sales and services provided are final once any chain is cut, and no return policy is applicable to any product purchased or service rendered.

2. I acknowledge that LIV BEE Jewelry Co. warrants that all products and/or services shall be completed in a workmanlike manner consistent with standards in the trade, profession, or industry and shall be fit for ordinary use, of good quality, and with no material defects. In the event of a material defect, I must contact Linked x Liv within 24-hours of purchase, to coordinate necessary repairs.

3. I acknowledge that my participation in attending personalized jewelry services entails known and unknown risks that could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I understand that such risks cannot be eliminated without jeopardizing the essential quality of service.

4. I expressly agree and promise to accept and assume all the risks existing in this sale and service.

5. I hereby voluntarily release, forever discharge, and agree to indemnify, defend, and hold harmless LIV BEE Jewelry Co. their agents, owners, officers, volunteers, participants, and other persons or entities acting in any capacity on behalf of LIV BEE Jewelry Co. (the *Indemnified Parties*) from any and all claims, demands, or causes of action, which are in any way connected with services rendered or products sold by LIV BEE Jewelry Co., including any such claims which allege negligent acts or omissions of LIV BEE Jewelry Co., and any attorney’s fees or costs to enforce or defend this Agreement.

6. I certify that I have adequate insurance to cover any injury or damage arising out of the products purchased or services rendered, and I agree to bear the costs of such injury or damage not otherwise covered by my insurance. I further certify that I assume the risk of any medical or physical condition I may have, whether known or unknown.

7. In the event of any litigation arising out of this Agreement, I submit to the jurisdiction of, and waive any venue objections against, the United States District Court of Virginia, and I further agree that the substantive law of that state shall apply in any such action without regard to the conflict of law rules of that state. I agree that if any portion of this Agreement is found to be void or unenforceable, the remaining document shall remain in full force and effect.

8. I acknowledge that the first repair of any piece of jewelry will be complementary in the event of any piece of jewelry breaking, any additional repairs will cost $20 per each repair with a max of 3 repairs per chain. Additional fees may apply.

By signing this Agreement, I acknowledge that if anyone is hurt or property is damaged during my participation in this service, I may be found by a court of law to have waived my right to maintain a lawsuit against LIV BEE Jewelry Co. on the basis of any claim from which I have released them herein.

I certify that I am at least eighteen (18) years old and am fully competent to give my consent. Furthermore, I have had sufficient opportunity to read this entire Agreement. I have read and understood it, and I agree to be bound by its terms.
